If so - 

 

To request an appointment ... would  you kindly:

1. have the tests ordered by your referring doctor (* if * any have been requested)
2. complete our online form (on the Form tab)
3. send us the referral from your referring doctor

  Once we have:
 

• the results of any tests

• your completed Patient Registration Form

• the referral from your doctor

      we will contact you for an appointment
